Internal requests for data, new reports, or updates to existing reports are prioritized and managed through the data request process. This form should be used for updates or reports that impact the day-to-day operations at DMPS.

Conducting Research or Surveying (DMPS Employees, Students, & External Researchers)

​The DMPS Assessment, Data, and Evaluation office reviews all internal and external requests to conduct research or survey in the district in compliance with local, state, and federal policies.
Who Should Complete this Form?
  • Current DMPS Employees conducting research of any type at DMPS
    • ​Including, but not limited to research required by coursework (BLUE/PURPLE/GREEN Contract Capstone)
  • External researchers who wish to use DMPS data in a research study.​
  • Any person or entity wishing to conduct a student survey in DMPS.​
